This is a form to register for vaccination against COVID-19 in Iceland. The vaccination is available free of charge to all who live, study or work in Iceland. See also https://www.covid.is/vax.

For primary vaccination the vaccine from Pfizer is used for all age groups. For booster vaccination the vaccines from Pfizer and Moderna are used. Nuvaxovid and Janssen vaccines are available on request or in case of contra-indications of mRNA vaccines.
An Icelandic National ID Number is not a requirement but you must register your name, date of birth, sex, nationality and type and number of identification document, such as passport or card from the Directorate of Immigration.
The vaccinations are done all over the country, you must specify in which part of the country you would like to receive the vaccination.
If you have already received COVID-19 vaccination in another country you need to register the date(s) and type(s) of vaccine for all doses received. To get doses formally registered in Iceland, for them to be included in the Icelandic vaccine certificate, the vaccination certificate from abroad must be sent to the primary health care service in Iceland. If a previous COVID-19 vaccination was done elsewhere you may use this form for either a second primary vaccination dose or a booster dose.
If you have a history of acute allergy (anaphylaxis) after an injection, for example another vaccine, you cannot be vaccinated unless a specialist in allergy medicine has reviewed your history and determined which vaccine if any would be most appropriate. Please do not use this form to register for the vaccination in that case, but contact the primary health care on www.heilsuvera.is.
Some people need to get the vaccine at very specific times. Please read the instructions carefully and arrange the most appropriate time for an appointment with your primary clinic.
